Other aspects that may be relevant to your patient are rehabilitation or end of life care, relevant government guidelines and research need to be linked to the patient. You will need to refer the NMC code in relation to the role of the nurse in Health promotion. Link to the patient in this case study.

4000 word Case Study: Learning outcomes 1-4
These learning outcomes can be understood in the following ways:
Learning Outcome 1: Identify the physical and psycho-social challenges that the patient in your case-study is experiencing. Challenges may include mobility, financial, transport, access to treatment, stress for example. Consider the definition of health, psychological and social needs, and the social determinants of health.
Learning Outcome 2: Consider various policies and drivers and how these are related to effectiveness of care and public health and long term/life shortening conditions. Examples of National Policies include National Service Frameworks, NICE guidelines, Care pathways, Framework for Personalised Care and Population for Nurses, Midwives, Health Visitors and Allied Health Professionals for example.
Learning Outcome 3: People with long-term and life limiting illness in the past have experienced fragmented care when care should be organised around their needs. Define integrated care. Discuss how integrated care benefitted or hindered your patient care.older people and those with long-term conditions, care is too often
Learning outcome 4: Identify and discuss the role of the nurse in preventing ill health and promotion of health and wellbeing within different health care settings, considering particularly the NMC, NHS constitution and Every Contact Counts and person-centred care.
